St. John is hosting the 2nd annual optional Flu and Updated COVID* Vaccine Clinic for students on campus during the school day on Friday, October 6, 2023. In order for your student(s) to participate, a parent/guardian must submit the sign up form below and complete and return the consent form via email to Dora Hall, dhall@st-johnschool.org, or a hard copy to the main office. Questions can be directed to Dora Hall, School Nurse, dhall@st-johnschool.org.

- Deadline to submit the sign up form AND complete and return the consent form is Monday, October 2, 2023.
*As of 9/21/23, due to supply issues, Safeway only has the updated COVID vaccine available for students ages 12 and older; however, this may change any day. We recommend that you sign up your child for the updated COVID vaccine as though it is available for students ages 5 and older on our clinic day (10/6), and we will provide updates as we get them. The updated COVID vaccine will not be available for children under the age of 5 on clinic day. The flu vaccine will be available for all ages.
