On-Campus Vaccination Clinic: Important Information

Volunteers Needed: We need volunteers to support the students and the vaccine clinic throughout the day on Tuesday. We have created various roles – check-in, getting students from class and emotional support/recovery – but we may shift things around depending on the needs throughout the day. Please sign up to help make this event a success! 

Important Information and Forms Needed for Vaccination Clinic 

  • The deadline to sign up for the vaccine clinic is tomorrow, Friday, 11/4. If you are interested in having your student participate, please email Dora Hall, ARNP at covidresponse@st-johnschool.org to be added to the list. Please include your child(ren)’s name, grade, and which vaccine(s) you would like. The updated Covid booster and flu shot will be offered. 
  • Each student MUST have a consent form signed and submitted by a parent/guardian in order to participate in the clinic. Consent forms can be emailed to covidresponse@st-johnschool.org or dropped off at the front office with Mrs. Q. Forms must be submitted by Monday, 11/7
  • For students receiving the Covid booster:  
    • Please put your student’s Covid vaccine card in their folder to bring with them on Tuesday.  
    • Reminder – in order to receive the booster: 
      • A child must be at least 5 years of age, and 
      • They have completed the primary series of vaccines (the first 2 doses of Pfizer/Moderna/Novavax or 1 dose of Johnson & Johnson), and 
      • It has been at least 2 months since the last dose (the last dose could have been a primary dose or booster dose)
