Back to School Parent Authorization Form

St. John Faculty and Staff look forward to welcoming students and families back to school next week! We hope you are excited about the upcoming school year too. The Back to School Checklist may prove helpful as you prepare for the new academic year.

Recall: families are required to review the handbooks/policies found on the Back to School Parent Authorization form and submit it by Tuesday, September 1. Submission of the form covers all students in the household and must be completed by the parent(s)/guardian(s).
